Home visit and full assessment, your Aging-in-Place Plan, a 90-day action plan with named owners, the one-page emergency sheet, and a family meeting to walk everyone through it.

Then choose the level that matches your situation. Flat monthly fee, month to month, cancel with 30 days' notice. No setup fee.
Steady
$375 CAD/month
For a situation that's stable, but that you'd rather not be watching alone.

Most Families
Active
$675 CAD/month
For a situation that's changing — new diagnosis, new services, or things starting to slip.
- Everything in Steady, plus:
- Hands-on coordination with providers
- Funding applications managed end to end
- One appointment attended each month
- Plan reviews at 30, 60 and 90 days
- Next-business-day response
Intensive
$1,050 CAD/month
For a complex or high-stakes situation, or a family carrying this from a distance.
- Everything in Active, plus:
- Full navigation across health, home and community
- Hospital admission and discharge support
- Scheduled family update calls
- Emergency plan kept current
- Same-business-day response
Not sure which one is your family? Most people start with the Roadmap and we recommend a level from there. If none of them fit, we'll say so.
What's not included: LCC does not provide direct caregiving, home support, or personal care services, and we don't make medical decisions on your behalf. We coordinate, advocate, and navigate — working alongside the people already in your family member's life.
Hourly & flat-fee work
These can be booked alongside a monthly plan, or on their own — no package required.
Single-session consultation
One hour by phone or video. A good starting point if you're not sure what you need — no commitment required.
Hourly coordination & advocacy
Research, provider calls, system navigation, and follow-up on your behalf. Billed in 15-minute increments.
Appointment accompaniment
We attend a medical or other appointment with you or your family member, take notes, and follow up on next steps.
Funding application support
One government program, subsidy, or benefit — we prepare and submit the application on your behalf.
Family meeting facilitation
We prepare, run, and summarize a structured family conversation about care decisions. Includes written notes.
Hospital discharge navigation
We coordinate the discharge plan, liaise with the care team, and make sure the transition home is safe and supported.
In-home project oversight
On-site supervision while a contractor, cleaner, or service provider works in the home.
A $99 deposit is required to book hourly work. It is applied to your first invoice.
Government Funding
Many families qualify for support that offsets the cost of care.
BC has income-tested programmes, tax credits, and subsidies most families don't know about. The Roadmap includes a full funding sweep — we find what you qualify for and apply on your behalf.

Is any of this covered by MSP or extended health insurance?
MSP does not cover care coordination services. Some extended health plans include a care management or case management benefit — it's worth checking your policy. We can provide itemized invoices to support any reimbursement claims.
Do we have to start with The Roadmap?
Yes. The Roadmap is how we get to know your family member's full situation — without it, we can't do the ongoing work well. It's also where the emergency sheet and 90-day plan come from, so most families find it valuable on its own.
What if our situation changes and we need a different level?
You can move between Steady, Active, and Intensive at any time with 30 days' notice. We'll usually be the ones to suggest it if we think a change makes sense — either up or down.
How does billing work?
The Roadmap is invoiced on booking. Monthly plans are billed at the start of each month. Hourly and flat-fee work is invoiced after each session, with a $99 deposit required to book. All invoices are sent by email and payable by e-transfer or credit card.
