We take the time to understand your situation fully before recommending anything. Here's what working with us looks like.

Our process — four simple steps

Step One

A conversation, not a consultation

We start with a free call — no forms, no pressure. You tell us what's going on, and we listen. Whether you're navigating a new diagnosis, recovering from a hospital stay, or simply noticing that things are getting harder, we want to understand your specific situation.

This call usually takes 20–30 minutes and can include any family members you'd like involved.

What to expect A warm, relaxed conversation. We'll ask about your daily life, your concerns, and what support might look like for you.

Step Two

A visit to your home

Your Living Care coordinator visits you at home — where you're most comfortable and where we can see your actual environment. We take time to get to know you, not just your medical history.

We review your daily routines, medications, home safety, social connections, and what matters most to you. Family members are always welcome to join.

What to have ready A list of current medications and any doctors' contact information is helpful, but not required. We'll guide you through the rest.

Step Three

Your personal care plan

We put together a clear, practical plan — your Emergency Information Sheet, a medication tracker, and a coordination plan tailored to your needs. Everything is reviewed with you before anything is finalized.

We connect you with the right services: home support, medical follow-up, transportation, community programs, and more. We do the research so you don't have to.

You're in control Every recommendation is a suggestion, not a directive. You decide what feels right. We're here to inform and support, not to take over.

Step Four

Ongoing care, ongoing connection

Your coordinator stays involved — checking in regularly, adjusting the plan as your needs change, and being a familiar face you can call when something comes up. Families also have a single point of contact for updates and questions.

As life changes, so does our support. We're built for the long term.
