A list of current medications if you have one — names and dosages. We'll help you document everything if you don't.
Doctor contacts
Your family doctor's name and phone number. Any specialists you see regularly are helpful too, but not essential.
Family involved?
Feel free to have a family member or trusted friend present. Many people find it reassuring to have someone they trust in the room.
Nothing else needed
There are no forms to fill in ahead of time, no preparation tests — just a comfortable space for a conversation.

We summarize what we heard, share any immediate suggestions, and explain what comes next. You'll always know what we're doing and why before we do it.
